CENPA Truck – There is a truck available that can be signed out in the front office.
You must complete UW Driver Safety and Awareness Training before you can use the truck.

Hours of Operation (Effective March 15, 2013):
7:00 am – 4:00 pm, Monday through Friday (They open promptly at 7:00 am and must have the gates locked by 4:00 pm—so arrive no later than 3:50 pm.)
- Pull up to the fueling island in the middle.
- This is a card terminal and there is a blue payment card on a chain.
- Swipe the blue card and you will be prompted for the following information:

Mileage:
Budget Number: 0 (Since this is a UW Fleet vehicle, they have the budget number to be charged, so just enter a zero.)
- Select the pump.
- Fill tank.
- When finished, flip paddle on pump to “Off”.
- No receipt is provided.
Fueling information will be sent to Fleet Services and the amount will be charged to our budget number.
Before you take the CENPA Truck, please review the conditions under which you are using this UW Fleet Services truck:
- You must possess a valid driver's license. You must also have completed UW's Driver Safety and Awareness Training. Once you have completed the training, provide Victoria with a printout demonstrating that you have successfully completed the course; this is needed for our records.
- You should check the fuel level before leaving with the truck, and if you notice that the fuel is low, please notify the CENPA Administrator. Out of pocket charges for vehicle service calls caused by the negligence of the driver are not reimbursable. Examples include: service charges for the delivery of fuel; retrieval of keys from locked vehicles; jump starting vehicles when the lights have been left on; etc. (RCW 4.2.2.1.4 D "Miscellaneous Travel Expenses") Fueling instructions are on the reverse.
- Operators are personally responsible for parking citations and moving violations issued while operating a University vehicle (WAC 478-116-520 "Motor Vehicles-Payment of Fines and Penalties").
- You agree to use this vehicle for official University of Washington business only.
- You agree that you will not drive this vehicle off paved roads. Operating a vehicle off road without prior Motor Pool approval may result in the suspension or loss of rental privileges.
- You understand that University vehicles shall not be used for transportation between a personal residence and the University of Washington campus or a University station.
- Smoking is prohibited in UW vehicles.
- The only authorized passengers are research subjects unable to transport themselves; patients and patient's families requiring transport for patient care service; vendors in the course of business; visiting dignitaries, speakers and scholars; disabled persons attending UW sponsored conferences.
- The following are NOT AUTHORIZED passengers: Conference attendees; spouses; children and acquaintances of staff, students and faculty; work experience program participants; recruits; prospective employees and or graduate school candidates.
- If you lose the key, a $20.00 minimum administrative fee will be assessed to the renting department to replace the key.
- If you are involved in any type of accident while the truck is in your possession, you must complete the accident report form that is located in the brown envelope in the glove box.
- You should briefly inspect the vehicle before taking the truck and upon returning the truck to the CENPA Truck parking space. Any damage should be reported immediately to the CENPA Administrator.
- If you notice any malfunctions while driving the truck, this should be reported immediately to the CENPA Administrator. (For example, if the parking brake handle comes off in your hand while you are releasing the parking brake, this should be reported so that it can be repaired.)
- Always sign out and sign in on the "TRUCK USE" Sign Out Sheet that is located on the front counter in Room 180. We are required to maintain this log to ensure continued use of the truck.
