If you or a co-worker has an on-the-job accident – or even a near-miss – you need to let the university and CENPA know. This way, we can be aware of the hazard, and take steps to make our laboratory safer in the future.
The university has an Online Accident Reporting System, or OARS. Go to this link to submit an OARS report. Ideally this should be submitted within 24 hours of the incident. Please also notify Doug Will, Hamish Robertson, and Diana Parno.
OARS requires a UW NetID and should be used for any work-related injury, illness, or near-miss, whether it involves a UW employee or a student, volunteer, or visitor. You can find more information about OARS here.
If the incident is serious – i.e. if someone is hospitalized or killed – a witness or responder should call EH&S right away at 206-543-7262. Outside of regular business hours (8-5 M-F), call UW police at 206-685-UWPD (that is, 206-685-8973). They will alert the EH&S worker on call. This call should be the first thing you do after taking immediate steps for the health and safety of those on the scene.
